Our mission at Pathfinder Care Coordination is to partner with Alaskans experiencing physical, intellectual, and/or developmental disabilities to receive the services and supports they need to live their lives in the most meaningful way.
We help our clients receive the guidance, access, and resources needed to meet essential needs, make choices regarding their care, identify and reach goals, and achieve greater well-being.

Pathfinder Care Coordination helps qualified individuals apply for and maintain Alaska TEFRA Medicaid and Home & Community Based Waiver Services. We serve Alaskans Statewide for TEFRA and focus on the Mat Su Valley Region for Waiver Services.
